Our Community Based Care Services

Our Community Based Care Team provides in-home case management for clients on the Community Choices Waiver and Supports Waiver, as well as Chronic Care Management Services for individuals living with complex health needs. The Wyoming Supports Waiver (WSW) is a Medicaid Home and Community-Based Services (HCBS) program that provides essential services to individuals with intellectual and developmental disabilities (I/DD).
